Please download and fill out this form to request an expense reimbursement or to apply a credit outside of Crossbar. Submit completed form and supporting documents to: reimbursements@luckypuckshockey.org

*** GOLF CREDITS INFORMATION ***

IMPORTANT, PLEASE READ CAREFULLY

Adult Lucky Pucks Golf credits will be distributed through Crossbar and applied directly to your account in the portal. Once your credit is applied, you will receive an individual email confirming the amount—this is your official Golf Credit. You may use this credit in Crossbar toward any available GNO or LP program (ie, Summer League, clinics, etc.).

If you prefer to apply your credit toward an LP tournament (FWHL or Nationals) or toward LP apparel, please email Maria (email only, please, for record tracking purposes). In this case, you will need to submit the Reimbursement Request FORM, and Maria will assist you with using your credit accordingly.

Please note that it is each individual’s responsibility to log in and use their credit, so be sure to access your account and apply it.
